Services are covered by Medicare, Medicaid, third-party (insurance), private pay, or Senior Living Program grant according to eligibility.
All services/cares are provided in the client’s place of residence.
Services are delivered per Plan of Care as developed in cooperation with the client, their physician, and agency staff.
The agency maintains strict confidentiality concerning all client information. Client approval is required prior to the release of any information.
Visits will last 15 minutes to two hours according to client needs.
All services/visits are normally scheduled for Monday through Friday, 8 a.m. to 4:30 p.m. If needs can better be met at other times or days, alternate scheduling can be made on an individual basis. A registered nurse is on call and available 24 hours a day, seven days a week.
All services are provided on an intermittent basis.
Performing a baseline assessment to assist in establishing a plan of care appropriate to meet each individual’s needs.
Examples of Skilled Nursing Services:
Medication: administration, set up and/or instruction
Ostomy care: Tracheostomy, Colostomy, Ileostomy, etc.
Discharge: planning and referral
Evaluation for waiver services
Home Health Aide
Home health aides follow a plan of care as established by their R.N. and physician.
Examples of Home Health Aide Cares:
Range of motion
Assist with exercises
Assist with ambulation
Perform/assist with personal cares:
All Home Health Aides have completed a 75 hour Home Health course and/or a competency test.
All Home Health Aides are supervised.
Covered by Waiver services or private payment and limited service available at reduced cost to low income seniors over age 60 though the Senior Living Program grant.
Examples of homemaker:
Respite available upon staff availability.
Therapy services are provided as per physician orders and as contracts are available.
Services available include physical therapy, occupational therapy, and speech therapy.
The agency will be responsible for coordination of all services and will maintain open communication with the client’s physician.
